Assuming you have a Lastfm account and have been scrobbling your tracks for some time then the system at Lastfm will have 'analysed' your music choices and identified several other folks that share some or all of your taste - these are designated as your 'neighbours'. So if you select play neighbours you will get a selection of music which has been scribbled by other people who share your tastes as defined by whatever algorithms Lastfm use to trawl through this stuff. I do this sometimes and it can throw up some interesting tracks that I might never have thought of listening to. And if you don't like what your neighbours do then you can skip a track and try the next suggestion.
The other similar option is 'Play Recommended' which cues up tracks that Lastfm think you might like. I can't really tell the difference between 'neighbours' and recommended' to be honest - either can be generally enjoyable and sometimes enlightening.
