Sounds like permission issues or the audio devices drivers not yet being initialized when squeezeslave is starting from init. Squeezeslave usually shuts down if it can't open the audio device. Can you confirm that the process is actually running? If yes, are you specifying the output device to use. Squeezeslave could be using another sound device.
