There's a problem with the server that under certain conditions it doesn't report changes if you do a clear&rescan. If the number of tracks, albums etc doesn't change it's a bit hard to know the library has changed. It's not a problem with "look for new and changed" because when you do that, the old library, especially the IDs stay valid.
