Most of the log shows you still have a network problem - timeout on
connections

This looks like you have lost the plugin that you installed back in

Is the PlayMP4 plugin installed ? - check WebUI Settings/Plugins   
If so what version does it show ?
what version of LMS are you running ?

What are the settings for MP4 in your WebUI Settings/Advanced/Filetypes
