This morning, following a safe boot, I deleted/reinstalled the LMS prefpane -- and all is working.
This certainly should not be necessary... are you using the Group Players plugin, or do you sync manually?
This is an issue I experienced for the first time a week or two ago, when I was working on an updated helper binary. I thought it might have been a problem with that new binary, but obviously it wasn't. What seems to happen is that LMS for whatever reason decides that the first track wasn't playable, thus skips to the next one. And here it repeats, skipping all tracks in a playlist. I've seen it happen with non Spotify tracks, too. But in the case of Spotty it's particularly bad, as each attempt at playing a track would launch a helper instance. I've never seen it happen with "real" players, but only in my testing with a local Squeezeplay.
What you could try to do in such a case is just kill all those spotty instances. "killall spotty" did help to bring LMS back to live...
