I also experienced the issues - volume suddenly set to 49% or increasing to 100%. iPeng was installed on my iPhone and iPad although I only used the Material Skin web UI. I had the suspicion that iPeng was causig the volume changes so I uninstalled the app on all devices. I had no volume changes since then so it might have been the cause in my case.
