I have seen this behavior and been able to fix it on my system. 

1. Download Tag&Rename (It has a 30-day free trial).
2. Browse to one of the files in question.
3. Double-click on the item to edit its tags.
4. Click on the Additional tab in Tag&Rename.
5. If you see the duplicate information in one of the fields, delete
it.
6. Clear and rescan your library in SlimServer.

Let me know what happens.
