I just had a similar problem and here is what I found out: It is a known bug/limitation of the Windows QT library that it does not handle filenames with more than 63 characters correctly!
In my case, the actual lenght of the file name does not matter -- problems only arise when there are two files in the same directory that share the first 63 characters. Too bad...
