I've just had something similar - in my case, upgrading to 6.2.2 turned back on "Use I-Tunes", and I-Tunes points to a shadow copy of my FLAC directory structure where the contents are transcoded to MP3s. Hence duplicate listings and etc. I wouldn't dive into SQL-browsing just yet, just find two duplicate tracks, click the track name for song info (I'm using the Fishbone skin, but assume others have this functionality), and compare the location info.
