What OS? Which version of SS, exactly? What file type? If MP3, might you have both v1 and v2 tags? Do you have any playlists? Using iTunes? One interesting (and easy) test would be to create a new music folder and just copy one or two albums into it. Make sure that you don't have any playlists in play. Then point SS at that folder as its library, turn on some debugging flags (d_scan at least, you could also search the forum for other suggestions), do a full clear and rescan and see what happens (look in the log file which is accessible from the debugging page).
