One small suggestion, and it pertains to many of us when troubleshooting a problem....
If you have to do a series of rescans to see if proposed fixes are working, it saves a bunch of time if you'll redefine the library so it's a tiny subset of your full library, like one artist or one genre. That way, rescans only take a few seconds. You can immediately see if the problem is solved, and if not try something else and scan again.
