Could be something corrupted (an index maybe) in the database. You could repair it with the right tools, but it's probably easier to just delete the whole database. Try shutting down SlimServer and deleting the MySQL database files. I'm not positive where that is on Windows - probably under C:\Program Files\SlimServer\server\Cache somewhere.
