Followed your steps (adjusting the paths appropriately for Vista x64)
but I didn't have any luck.  It doesn't seem that this is causing a
music re-scan since the server knows the library size when it starts
and doesn't seem to take the cpu cycles of a rescan.  So I wonder if
I'm not doing it right, or there's a step missing.

I did find my problem though.  When I poked around more I noticed there
were also some duplicate tracks listed in some of the albums which
suggested something weirder.  On a whim, I disabled the .cue extension
for the audio file scanner (playlist scanning was off anyway).  After a
re-scan with that change, no duplicates and no more bad first tracks.

The .cue sheets were generated by AutoFLAC and have some quirks that
make them a bit non-standard.  I guess looking at them as well was
misleading the scanner.
