A little more background on your set up would help but a couple of comments from my experience. First of all the standard namings that SC looks for are "cover.jpg", "albumartsmall.jpg", "folder.jpg", "album.jpg", or "thumb.jpg". If you want to use an alternative to one of these you can set that up via the Advanced Settings tab. The normal place to locate the image files is in the same directory as the album tracks for that album.
Secondly you should make sure that you have done a full "Clear and Reascan" to ensure the database has grabbed all of the artwork. Finally, I did have some issues myself with cover art recently - not that it was missing but that it was being wrongly associated with incorrect albums. The solution in my case was to update to a newer nightly which fixed it.
