There is a good chance that this issue is caused by the format of your
playlists.  Open one of the playlists that contains a duplicate song up
in a text editor like EditPad (Notepad will do the job, but isn't that
user friendly).

What are the paths to the playlist files like?  Are some of the paths
different from the way that SC sees your music files?  E.G. if SC sees
your Music Library as \\Mediaserver\Music\song.mp3, do some of your
playlists refer to the same files as M:/Music?  If so, SC will think
that the files using the "M:/Music/song.mp3" (in this example) are new
and different files, not existing files which are already in the
Library.

The solution is pretty simple too.  Use Find and Replace in your text
editor to change the file paths to reflect the format that you have
entered into your Library Settings in SC (i.e
\\Mediaserver\Music\song.mp3 format). 

Then do a full clear and rescan.
