It's not an area of software I've ever played with, so I don't know, but what you've described about how you want to do this reminds me of Joel Spolsky talking about FogBugz in one or more of his newsletters. It's payware, for Windows, but you might want to look into it; starting: http://www.fogcreek.com/FogBugz/ There's "Try it"/"Buy it", where the buy option includes hosted provisioning or downloading the software to run yourself. Per-user licensing.
