Scott and all, It's bad especially with remote apps. You can play
around with cursors and terminal types, but I've not found a way to
work with it in better fashion than you describe.

one area where VO doesn't do as well is Terminal. It seems that even
when interacting with the Terminal window, its very difficult to get
feedback especially when I'm using ssh to connect to a remote box.
What I mean by this is that sometimes when using VO keys plus the up/
down arrows, the last line will not speak until I press enter as
though the last line is hidden which is possible, I do not know. Also
sometimes if pressing up arrow with the VO keys, its as though the
cursor has shot clear to the top of the screen and it begins reading
everything from the top down. Also if editing files remotely with
lets say vi, its very tricky to get the cursor in the proper
location. Now not saying its just bad, but what I'm saying is that I
find Terminal a little tough to work with and especially while
connected remotely. Now perhaps there are some things I can do with
Terminal or VO to improve the experience and if anyone has some
suggestions, I'd gladdly try them out.
Of course I'm used to the shell on my Linux box using Speakup and so
of course its not going to be exactly like access on the Mac.
