It's interesting. I  rarely have experienced this quiet dock thing until
It came up on this list today.  I have had instances in which switching
between active windows has produced only a clicking noise and no speech.  I
usually just switch VO off and on to solve it.  That worked for the Dock
today as well.  To me, though, it's better then the screen reader entirely
hanging up and you have to fight with the alt control delete mess or just
reboot the whole system.

In my experience, the quiet dock phenomenon can also occur when there is no
popup or undismissed alert, etc.
