So are you saying uncheck both of the mouse tracking options in VO utilities? So VO doesn't track the mouse, and the mouse doesn't track VO?

I would make sure the mouse is not following any other cursor and, after routing it to the VoiceOver cursor, don't use the keycombinations. Simply reach for either your mouse or track pad and physically click the mouse itself. This always works for me.
