Others responded to your first item so for your second item you said:
sometimes cmd+l to open a web location will cause Safari to quit.
There's no dialogs, no errors, no crash reports. It just quits and
that's it, reminds me of the behavior I get in Linux when a GUI app
segfaults (though in Linux at least I can see an error message in the
status console). This is the more annoying behavior by far, as I have
to play around a little to get it working correctly again. Sometimes
moving to the address bar with the VO cursor will fix it, sometimes
not and I have to unlink the cursors and then click the mouse on the
address bar.
Has anyone seen either of these behaviors before or did I mess
something up somehow? I'm wondering if it might be time to give my
Mac an OS reinstall.
these kinds of issues do not require a reinstall. Pretty well the
only time you need to do a reinstall with OS X is when it is totally
unusable. that is to say, you can't log in, total crashes of the OS
(which seldom if ever happens), and some hardware issues that affect
the boot sectors. Probably not much else though.
With respect to your problems with Safari. I have noticed some
issues with safari when using the cmd+l command when the Address Bar
is visible. VO seems to jump around a lot and occasionally VO will
restart itself but I've never had Safari actually quit on me. You
can hide the Address Bar from I believe the View menu or simply press
cmd+shift+back slash
