Have you tried the command-f5 key to try turning voiceover on again?
If it's off, when you reboot, it remains off. You'll need to turn it
on again. One way to tell if your account is broken, is to see if it
works at the login window, but not at your desktop.
And of course if yu had another user, and it worked there, then you'd
know it was your account causing the problem.
However, since you didn't get the other account created, that option
is kind of out.
But, if it works at the login window, and not in your account, then
you know as well.
Give those a try and see what happens.
