Well, it took some fiddling, but I got it to work the way it's supposed to, at least with Audacity. iiTunes is another matter, but I think that's more because it's iTunes than anything else.

If I'm understanding you correctly, I would press cmd+2 to ensure that I'm in List mode in the dialog. Then when you interact with the File Outline, navigation works much smoother. You can use VO keys+vertical line to open/close folders and arrow through the rest. the "Browser" tends to be related to "Icon" view which is often unpleasant with VO.
