I don't know of a way to make iTunes download all podcasts since
the last time it checked the feed.  You could put in a request for
Apple to support such an option.  This is a problem particularly
for laptop users, since if multiple episodes get posted between the
last time you're connected you'll only get the last one automatically
downloaded and have to manually download the others.  You could
have iTunes check the podcast feed hourly instead of daily, but that
would only help with multiple episodes posted while you were 
connected.

I also sometimes see deleted podcasts get downloaded again, but
this mainly seems to happen with the recent versions of iTunes.
When there were fewer features this never happened *grin*.
Sometimes this happens because the way the authors have set
up their podcast feed.  One NPR program used to automatically
generate an entry for the "latest" episode by reading some web
location, and when it got "stuck" you just saw the same episode
being repeated as the "episode for that week" for a few weeks 
until they fixed the problem.  It's safe to delete.
