Do you have your safari preferences set up to tab browse?
Realistically, I think every user will have their own style of setting
up preferences in both VO and Safari, but in the Safari Preferences
under tabs, make sure the, select tabs and windows as they are
created, is checked, and check the item under advance that is, press
tab to highlight each item on a webpage. Even if you aren't using
groups with VO preferences, as Josh pointed out, you might be
interacting with specific groups on a page by accident. Again, it's
hard to say, as web design is not at all a standard kind of thing and
can be a plain adventure no matter what browser or platform you are
using.
