Jed. Have you checked to see which apps are running? I've found
recently that sometimes when I bring up the voiceover utility, the
focus is not automatically taken to the new window, but if I
command-tab around, I find that the voiceover utility is actually
running. It's just that it didn't automatically take the focus into
that window.
