This is a known problem, and has been brought up several times. As of this moment, there does not seem to be a fix. The problem is that the iTunes store window, sort of like a mini web browser inside iTunes, does not expose its content to VO. Currently, you have two options for reviewing your account: 1. Fire up a windows PC or a VM, install iTunes and review your account with a windows screen reader. This is a bit tricky, but it's doable. This is the way I did it, if you need any help with doing this just ask. As reviewing your account only needs to be done once (unless you need to change some details later) you can easily forget about windows iTunes after this if you don't intend to use it. Signing into the iTunes store, once your account is reviewed, will work fine, as will authorizing your computer. Note that currently the only thing we can buy on the iTunes store is a single item, i.e. one song at a time, one audiobook at a time, etc. Personally, I prefer the amazon mp3 store, as the prices are pretty much identical to iTunes, their OS X version of the mp3 downloader is perfectly accessible, and you get actual mp3s, no DRM.
2. Get sighted help.
I've written to apple repeatedly about this one, and I'm sure pretty much everyone on this list has, too. Hopefully they'll address it; they did make smart playlists accessible, after all, whereas previously we couldn't work with those.

Btw, while we're on the subject of iTunes, anyone know how to access the iPod syncing preferences and such with VO? I can't seem to find those either. I just got an iPod classic and while I can put songs on it easily enough via Automater, I can't manage its preferences, make it sync with my library (I really want to do that), or manage the songs on it. I've tried everything, even having VO speak what's under the physical mouse and using that (I've gotten rather good at that lately) as sometimes it reveals things that VO normally can't see. But I keep coming up with nothing. Anyone figure this out, or is there a 3rd-party app that can set these?

I have my Apple ID and want to sign into the iTunes store. iTunes tells me that this ID has not yet been used with the iTunes store and that I must review my account information before continuing. I have clicked on the review button every way I know how, but all I seem to get as a response is to be placed back in the main iTunes window with the sources and song tables. Talk about frustrating. :-(
