memory stick

OK can someone help
i am trying to format a memory stick and every time i do i get an  
error message. it worked just fine as a fat formatted stick and could  
be used in both windows and on the mac. i wanted to format so it would  
be used to boot a Mac but now i can't read it in either windows or the  
mac.

any thoughts?
