For a really good Mac program to do this, take a look at Audiobook Builder from www.splasm.com. It's $10 and will do what you want, and even put chapter marks in the m4b files. Note though that M4B files with chapter marks have a maximum length of approximately 12 hours, so in that case multiple m4b files will have to be created. This is something Audiobook Builder will handle for you automatically. Oh and it's perfectly accessible. I don't know what programs will do this on Windows, though and I haven't bothered to find out since I don't use it.
