Is there a way to reassign a voiceover / mac keystroke? For example, I'd like to reassign the next paragraph command in Voiceover to VO + Control + down arrow.

Also, has anyone realized how compllcated Voiceover would be for an individual with manual dexterity problems, particularly if they only had use of one hand)?

The simplest way to reassign VoiceOver functions is to use Numpad Commander and take advantage of its customizable options. This is actually one of the methods employed by folks who are limited to one handed use, along with other Universal Access options such as Sticky Keys for multi-key inputs and Mouse Keys for fine control of cursor movement. The Numpad Commander assignments, along with all personalized VoiceOver preference settings (for voice, rate, verbosity, etc.) can then be exported to a memory stick by using the portable preferences option under the File menu on the menu bar of the VoiceOver Utility. They can be used with other Mac computers. The problem for laptop users is that Apple removed the numlock key and embedded numeric keypad capability on MacBook and MacBook Pro models manufactured shortly after the release of Leopard, which introduced Numpad Commander. So this method requires recent laptop users to use an attached numeric keypad to take advantage of Numpad Commander.

You can also lock your VoiceOver keys.

You may not need to reassign keystrokes, since you also have access to the regular Mac OS X keybindings that work with all Cocoa Applications (e.g., TextEdit, Mail, Preview, etc.) These are like emacs keybindings.

However, take a look at Greg Kearney's post about customizing your keybindings:
