Jonathan,

The lack of placement control for endnotes and footnotes is a known problem. 

The bibliographic project has proposed that footnotes and endnotes be enhanced 
to allow greater user control of their placement. For example the Chicago 
Style suggests the following arrangment for backmatter (the stuff after the 
main text) -


Appendix 
Endnotes / Footnotes
Glossary 
Bibliography 
Index 
Colophon 

OOo currently places endnotes and the very end of the document.

In writing these proposals we did not consider Endnotes without a page brake, 
but it should be simple to allow the user to select placement and turn page 
braking on or off.
