I confess that I haven't read every post in this thread, but my
interpretation of the subject is a little different from the binder type
approach.

To me it should be possible to have one document that can be:
  * printed nicely on pages
  * projected appropriately for a presentation
  * displayed on a web page
  * have interactive features (x forms?)
  * be spoken aloud for blind people or available over a voice phone

Take for example lecturers preparing material for lectures. They may
have a "book" style handout, they may have a presentation, they may have
on-line static information pages, they may have an interactive test on
the subject matter. Currently this would typically be handled by
separate files. Each year they update their material, and this means
making sure that each of these files is updated in a consistent manner.

All of this should be possible in one document through styles.

For the closest approach to this that I am aware of there is Course
Genie for an other office suite.

I have toyed with the idea of writing something similar for
OpenOffice.org, but it would be even better if styles in OOo were
enhanced to allow for this directly.
