So we did a heap dump of the server and found the TunnelService instance - and it's extensionsTunnel was still set to true.
Oddly - when looking at the registered exceptions for media type mappings, we didn't find any of our custom ones we've added - which may or may not be related to this issue ( content negotiation failed for our custom media type extensions as well as html/pdf).
