Do you mean to run on your own site? Or that might be hosted elsewhere? In the latter case, are you limiting your choices to CFML?
As for the former, another question then is how many surveys you want to do, how many questions you want to ask, how many respondents you expect, etc. Those all factor into the choices available. For instance, when we did a survey last year to find the most favorable location for the CFUG when we had to move, I used http://www.surveymonkey.com/. It's a standard in the field, and even lets you use it for free for surveys of no more than 10 questions and not more than 100 respondents. For many, that's all they need.
