Finally had to reboot the pfSense box protecting our company's main web
server.

It was up for 132 days using version 0.59 BooeyBooey.

For whatever reason, it wasn't doing 1:1 NAT anymore.  It was responding to
the IP hits itself rather than NAT'ing it to the appropriate internal IP.
Everything else was working correctly (outbound NAT, IPSEC tunnels, etc).

Sooner or later, I'll get around to upgrading it to a newer version.  LOL!
