I've been running Trixbox behind pfSense for over a year now.  I even have 
pfSense doing VPN to remote offices so extensions there can connect to the 
Trixbox for secure internal calls between extensions.

Your configuration sounds about the same as mine as far as the pfSense config 
other than I have both TCP and UDP open in my forwarding rules.  SIP uses TCP 
in some cases so you may need to have that open also.

Also make sure you have

externip = <your external 1:1 IP>
nat = yes

in the /etc/asterisk/sip_nat.conf file on the Trixbox.
