I'm using a Dlink POE 10/100 switch. VOIP and data are on the same network.
I believe the problem is either with the drivers for the 3c905 series network cards, or a bad config file. I re-created the pfSense config file from scratch, and the problems seem to have gotten better, although not perfect.
