Thank you for taking the time to report this bug.

I managed to reproduce it by running:

ditac test.html map.ditamap

(but, strangely enough, it does not occur when running:
ditac test.pdf map.ditamap ).

This bug should be fixed in next release of ditac which should support 
duplicate topicref entries, with or without a copy-to attribute.
