Yep, page headers/footers can be very complex. Apart from first/odd/even
there are sometimes requirements to have special header/footer for blank
pages -- if page is inserted at the end of the page sequence to produce
even number of pages.

Also sometimes different headers/footers are needed for index and ToC,
and...

If you want to be prepared for majority of possible requests you can
look inside DocBook XSL stylesheets -- but code for dealing with
headers/footers is one of the most complex and not easy to grasp :-(
