A while back, I wrote you about an issue I was having with the
page-ref-before parameter (see below).
I implemented your suggested bug workaround, and it worked fine.
However, I just upgraded to a Windows 7 machine and modifying the
xslutil.conversions
file per your instructions no longer fixes the problem.
 
I attached a copy of my modified conversions file so you can see it. I
also attached
a PDF showing the resulting "on page" issue.

I'm sorry but you have found a bug in XSL Utility. We'll fix this bug
in
the next release of XSL Utility.
 
For now, the only workaround we have found is to manually edit the
xslutil.conversions XML file generated by XSL Utility when you
customize
a stock conversion.
 
xslutil.conversions is found in the following folder on Windows XP,
Vista and 7:
 
%APPDATA%\XMLmind\FOConverter\
