Then DITA-OT world is certainly missing some knowledge. It was always
possible to generate index with FOP. Only trouble was (and is) that FOP
doesn't have extension (and doesn't support corresponding XSL-FO 1.1
feature) for merging page numbers. So it can happen that for some index
entries same page number appears twice in the index.

But even for FOP this can be overcome by doing some postprocessing, see:

DocBook stylesheets support this for ages, see for example
