Then DITA-OT world is certainly missing some knowledge. It was always possible to generate index with FOP. Only trouble was (and is) that FOP doesn't have extension (and doesn't support corresponding XSL-FO 1.1 feature) for merging page numbers. So it can happen that for some index entries same page number appears twice in the index.
