I've got it working. What appears to be the problem is the path I've
used, more precisely this: Program%20Files%20(x86) - the ( ) seems to be
the problem. At least when I use a path like {{image(file:/C:/Program
Files/Common Files/icon_beta.png)}} it works.
I don't know if this makes sense to you, but now you know.

I'm sorry but I cannot reproduce the problem you describe.
Before answering your email this morning, I've taken care to re-test
this feature using XMLmind XML Editor 5.6 running on Windows XP. I
used "Options|Customize Configuration|Change Document Conversion
Parameters" to do this:

That is, I didn't edit any configuration file by hand.
After your second email, I have re-tested this feature but this time
using header-right and a PNG file, and as expected, it works fine. See
attached screenshots.
Are there any limitations in graphic-file size, e.g. pixel-width/height?
The graphic-file is very small, measured on all parameters.
No. Even the very existence of the graphics file is not checked.
