I'm usually aiming to deliver WebHelp based on DITA source, which works

output .pdf instead. I can then edit the result with Acrobat Pro and add

prepared as a .pdf outside XML Mind. Can you suggest a more integrated
approach, or shall I carry on the way I've started?

You should give XSLT parameter title-page=<URI of a hand-written XSL-FO
file> a try.
