For now, the Index 'ToC line' (which is just an A-Z line) is not generated at all when ebookc finds an index heading which does not start with letter A to Z.
We'll try to fix the "Index creation issue for Scandinavian languages" bug, but it's unlikely that we'll fix this other one. That is, you'll probably never get an Index 'ToC line' for Norwegian Nynorsk, Norwegian Bokmål, Russian, Greek, Japanese, Arabic, etc.
