I've seen something similar under FastCGI; it only happens when
`APPEND_SLASH` is true, and it seems to be something to do with
`PATH_INFO` not being passed in properly. I've had this come up and so
have some other folks I've talked to, so there's definitely an issue, I'm
just not sure where it is.
