Took some custom SQL, but this works well:
content = Content.objects \
.filter(approved__isnull = False) \
.extra(where=['id not in (SELECT content_id FROM
relationship WHERE user_id = %d and "hasRead" = TRUE)' % user.id]) \
.order_by('-created')
Phew.
