Something funky went on with this for me too. I first thought it was broken a while ago when vbox moved to 2.10. But as soon as I let it convert all of my xml files to the new format, the plugin worked fine again -- so I figured it wasn't an issue with the plugin.
Don't worry about the icons. I'm working on it right now. It should be as easy as adding the new icons to the resources of the assembly, if I remember correctly.
