Worked with Jakub and determined that the plugins created by the 'Add to
Search Bar' Firefox extension were the primary source of issues here.
The OpenSearch plugin has been updated to handle those and was released
as part of Do 0.8, so I'm closing this issue. In the future, any bugs
reporting issues with resolving Open Search plugins should provide the
output of Do run with --debug, as the plugin now reports useful
information for debugging this type of problem.
