(This is just a sample because it produces a lot of these errors).

 

I do not know if this is a related problem, but, I also cannot add
users.  Whenever I try to add a user get to dialog boxes on says
"Password" and the other one says "The username 'josh' is already used
for somebody else. Please choose another username". But this is the
first user that I have added.

That sounds seriously screwed.  Try deleting docbookwiki_guide and
importing it again, and note any errors.  That would be using
content/clean.sh for the first step and content/import.sh for the
second step.
