I am in the midst of updating my "documentation process" document
(http://www.e-smith.org/docs/docprocess.html ) and should have a new
version online later today that gives more details and talks about the
XML way of working with DocBook.  The current one only talks about SGML.

Nope.  As far as I know there is no need for a license.  Actually, now
that you ask, I'll check into it.  People in the open source and DocBook
world are generating PDFs all the time and I have never seen any questions
about licensing.  I'll let you know.
