No, it's got nothing to do with extensions. All versions of Saxon
prior to 6.4.2 had a nasty bug that caused xsl:apply-imports inside of
a stylesheet that used xsl:include to sometimes fire the wrong
templates.
Extensions are still required for callouts (using programlistingco or
screenco), numbered program listings, and some table column-spec
calculations.
